Appointment Scheduling Information:

• Telephone Appointment Hours: Our staff is available for telephone appointments during the following periods:

•   Monday- Thursday  ** 8:30am to 5:30pm

•   Friday                      ** 8:30am to 4:30pm

• No telephone appointments scheduling is available on holidays or weekends. 

• ALL EMERGENCIES SHOULD GO TO THE NEAREST EMERGENCY ROOM.

Appointment Cancellation

• A 24 hour notice of cancellation is appreciated so that other patients may be scheduled for that appointment time.

• Your courtesy call is much appreciated.

Other Items Required for Appointment Efficiency.

• Bring your Insurance Card

• A referral is required from primary care doctor for HMO patients and is necessary to insure reimbursement and confirm your appointment.

• All Co-payments are due at the time of service.

• Patients under 18 years of age must be accompanied by a parent/guardian or have a signed waiver.
