Request an Appointment:

• Telephone Appointment Hours: Our staff is available for telephone appointments during the following periods:
   

•   Monday- Thursday  ** 8:30am to 7:30pm

•   Friday                      ** 8:30am to 4:00pm
   

• No telephone appointments scheduling is available on holidays or weekends.
   

• Appointment requests can be submitted by E-mail at anytime by clicking below; staff will confirm e-mail  appointment requests by telephone on the next business day.
   

• ALL EMERGENCIES SHOULD GO TO OR CALL THE  GRANDVIEW HOSPITAL EMERGENCY ROOM

Appointment Cancellation

• We ask for a 24 hour notice of cancellation so that we may schedule patients that may need your appointment time.

• Your courtesy call is much appreciated.

Other Items Required for Appointment Efficiency.

• Bring your Insurance Card

• A referral is required from primary care doctor for HMO patients and is necessary to insure reimbursement and confirm your appointment.

• All Co-payments are due at the time of service.

• Patients under 18 years of age must be accompanied by a parent/guardian or have a signed waiver.

• Patients who can NOT understand or converse in English should bring an interpreter; we have no language capability in the office.
