It is important to remember that participation by the practice means that the practice physicians have an agreement with the insurance carrier to limit our reimbursement to an agreed upon amount irregardless of our charges.
The insurance is the patient's insurance and referrals and authorizations may be required. Additionally co-insurance and co-pays may be required of the patient.
If you are not aware of your coverage it is recommended that you contact your primary doctor of your insurance carrier. Our receptionist will provide some assistance.
CO-PAYS AND CO-INSURANCE WILL BE REQUIRED AT THE TIME OF SERVICE. THERE IS A $10 CHARGE IF CO-PAYMENT IS NOT PAID AT THE TIME OF SERVICE>
